Creating a Custom Base Rate for a Facility or Patient

Follow these steps to add a new charge category for billing A0999 codes, which are used in place of ALS/BLS base rates. These codes are typically applied when billing for services such as lift assists, standby coverage, educational visits, and similar activities provided to facilities like nursing homes, rodeos, and high schools. Admin permissions are required to complete these steps.

The following procedure is intended to help you replace the base rate. If you need to create a charge that will be used in addition to a base rate, such as late fees, returned check fees, etc, see Itemizing supplies.

  1. Click Settings on the Billing navigation bar.

  2. Click the Charges tab.

  3. Click Add a charge category.

  4. In the "Add a charge category" window, enter the charge name and start date. If this category is for a limited time, enter the date the charges will expire.

  5. Click Continue.

    The page displays a list of charges for which you can change the description and set the price per unit.

  6. On the new charge category page, locate the A0999 HCPCS that is under the "Base rates" heading and then change the A0999 base rate’s ‘Description’ to whatever text you want the billers to see or to print on a facility invoice, like "Lift Assist."

  7. Click Save.

  8. Ensure that the new custom base rate name displays in the View field and then click the edit pencil in the top right corner.

  9. Update the price per unit for A0999 – if the fee differs by call or facility, leave the price per unit blank (if you want to enter a static fee amount that differs per facility, consider creating a specific charge category per facility).

  10. Click Save.

Once created, you can use this new charge category on the Claim:

  • For the level of service, select A0999 - Unlisted ambulance service.

  • On the Charges tab, select the charge category you created.
